Our goal was a descriptive delineation of these concepts at successive phases following LT. The practice of utilizing split liver grafts can potentially amplify the availability of liver transplantation (LT) to adult patients, especially in instances where the graft is divided between two adult recipients. A conclusive answer regarding the comparative risk of biliary complications (BCs) in adult recipients undergoing split liver transplantation (SLT) versus whole liver transplantation (WLT) is currently unavailable. A single-center, retrospective investigation of deceased donor liver transplants was performed on 1441 adult patients, encompassing the period between January 2004 and June 2018. SLTs were administered to 73 patients. The graft types utilized for SLT procedures consist of 27 right trisegment grafts, 16 left lobes, and 30 right lobes. The results of the propensity score matching analysis demonstrated that 97 WLTs and 60 SLTs were included. SLTs had a significantly elevated rate of biliary leakage (133% vs. 0%; p < 0.0001) when compared to WLTs; however, the occurrence of biliary anastomotic stricture was similar between the two groups (117% vs. 93%; p = 0.063). The prognostic value of acute kidney injury (AKI) recovery patterns in the context of critical illness and cirrhosis is not presently known. Our research aimed to compare mortality rates according to diverse AKI recovery patterns in patients with cirrhosis admitted to an intensive care unit and identify factors linked to mortality risk.
A cohort of 322 patients exhibiting both cirrhosis and acute kidney injury (AKI) was retrospectively examined, encompassing admissions to two tertiary care intensive care units between 2016 and 2018. Recovery from AKI, as defined by the Acute Disease Quality Initiative's consensus, occurs when serum creatinine falls below 0.3 mg/dL below baseline levels within a timeframe of seven days following the onset of AKI. Recovery patterns were categorized, according to the Acute Disease Quality Initiative's consensus, into three distinct groups: 0-2 days, 3-7 days, and no recovery (AKI persisting beyond 7 days). This quality improvement study, incorporating an interrupted time series analysis, drew its data from a longitudinal cohort of patients in a multi-hospital, integrated US healthcare system. With the aim of motivating frailty evaluation, surgeons were incentivized to use the Risk Analysis Index (RAI) for all elective patients from July 2016 onwards. The BPA's establishment was achieved by February 2018. Data collection was scheduled to conclude on the 31st of May, 2019. From January to September 2022, analyses were carried out.

Frail patients, through these referrals, gained a survival advantage equivalent to those observed in Veterans Affairs health care settings, which further supports both the efficacy and broad application of FSIs incorporating the RAI.
